How to Create Google Forms for Surveys and Quizzes [ 2021]

Before going to create the form, learn the following 6 icons which you can see in the side of each question boxes:

1] First icon - It is used to provide space for next question.
2] Second  icon - It is used to import question which is already created in your previous forms.
3] Third icon - It is used to add title and description.
4] Fourth icon - It is used to insert image from your PC / laptop / mobile
5] Fifth icon - It is used to insert you tube videos
6] Sixth icon - It is used to add section. [ For example: If you want to ask questions in 3 different sections, you can use this.] Sections are similar to roman numbers in question paper.

STEPS TO CREATE GOOGLE FORMS

METHOD 1

Step 1: Type https://docs.google.com/forms/ in your google search.
Step 2: Choose blank form.



Step 3: Click untitled form and delete it. Enter your title.
Step 4: Add form description below the title. [In form description, you have to type the motto of this form.


Step 5: Go to next box and enter your question.
[You can insert image for each questions and options, if you want.]
Step 6: Now, you have to choose answer format according to your question by clicking the icon on the top right corner of each question.

Let's see what are the types of answer format available:



a] Short answer: It is used to type answers which are in one or two words. For example, name, email address, etc.,
b] Paragraph: If the response was too long, then choose this option. For example, pros and cons of my blog.
c] Multiple choice: It is used in multiple choice questions.
d] Checker box: It is similar to multiple choice type but here participants can enter more than one response.
e] Drop down - By choosing this, you can create your own options and also helps to shuffle the options.
f] File upload - By choosing this, participants can upload their files after signing in to google. You can also choose the range of file size and number of files.


g] Linear scale - If the answer is about scaling, then you have to choose this option.
h] Multi choice grid - Here questions are typed in rows and options are typed in columns. It is mostly used for ranking. For example, to rank something as good / moderate / poor.


i] Trick box grid - It is similar to multi choice grid but here participants can choose more than one option.
j] Date and time - It is used if the expected answer is about timing.

Your google form will be automatically saved. There is no separate save icon. Before leaving the page, don't forget to go for preview.

METHOD 2

Step 1: Type https://docs.google.com/forms/ in your google search.

Step 2 : If you have no time, then choose templates. Here you can edit the ready made forms from template gallery which is present in the top right corner.
Totally there are three categories of forms in template:
1] Personal
2] Work
3] Education
According to your need, you can choose.
